You will need the following items to complete online enrollment:

• Phone number and address for emergency contacts

• Information about the last school attended, if any

• Enrollment Documents

You will have the option to upload the following required registration documents as part of online enrollment or you will need to bring them to the school:

Provide 1 item from each category:

1. Student's proof of age and identity
   • Birth certificate
   • Statement of the child’s date of birth issued for school admission purposes by the Vital Statistics Section of the Texas Department of State Health Services
   • Passport
   • School ID card, records, or report card
   • Military ID
   • Hospital birth record
   • Adoption records
   • Church baptismal record
   • Any other legal document that establishes identity
2. Health Information
   • Immunization/Shot Record
3. Guardian Information
   • State-issued photo ID
   • Driver’s license
   • Military ID
   • Passport
4. Proof of Address
   • Utility Bill
   • Lease Agreement

• Complete and submit your application for enrollment.
